Clarify that the state is a type-indexed map, as suggested by #26484.

Fix incorrect markups in the User's Guide
* Correct markup for C--: "C-\-" in reST
* Fix internal links
* Fix code highlighting
* Fix inline code: Use ``code`` rather than `code`
* Remove extra backslashes
