 Fix mkGadtDecl does not set con_forall correctly

 A GADT declaration surrounded in parens does not det the con_forall
 field correctly.

 e.g.

 data MaybeDefault v where
     TestParens  :: (forall v . (Eq v) => MaybeDefault v)
