Regarding other uses of `substTyUnchecked`, there is already the existing bug #11371, which was left open to address them (see https://ghc.haskell.org/trac/ghc/ticket/11371#comment:51). I count at least 35 calls to `substTyUnchecked` (run `grep substTyUnchecked */*.hs` in the compiler directory); I'm not sure if it's worth listing them all in the bug.
