 rts: Add FALLTHROUGH macro

 Instead of using the GCC `/* fallthrough */` syntax we now use the
 `__attribute__((fallthrough))`, which Phyx says should be more portable
 than the former.

 Also adds a missing fallthrough annotation in the MachO linker,
 fixing #14613.
